MD Health Center Membership / Health Card

These Terms & Conditions govern the use of the MD Health Center Membership Card and its associated healthcare services. By using this card, the cardholder agrees to comply with the terms outlined below.

1. Validity of the Card

• The MD Health Center Membership Card is valid only for the first month (30 days) from the date of issuance.

• All benefits and discounts must be availed within the validity period.

• Services not utilized within the valid period will not be carried forward or refunded.

2. Services Included

The membership card includes the following services, subject to medical necessity and physician approval:

1. Annual Comprehensive Physical Examination

2. Initial Blood Work-Up (No Extra Charges)
   Includes:

   • CBC

   • RFT

   • LFT

   • HbA1c

   • Lipid Profile

   • Urine Examination (U/E)

3. Unlimited Follow-Up Consultations

4. Unlimited Online Consultations

5. Radiology Services – 20% Discount

6. Repeat Laboratory Tests – 30% Discount

7. Specialist Consultations – 20% Discount

3. Usage Conditions

• The card is non-transferable and can only be used by the registered cardholder.

• A valid original card must be presented at the time of appointment or service.

• Online consultation benefits are subject to doctor availability and appointment scheduling.

4. Appointments & Scheduling

• All services are provided by appointment only.

• Appointments can be scheduled via phone or by visiting the clinic.

• MD Health Center reserves the right to reschedule appointments due to medical emergencies or unforeseen circumstances.

5. Medical Discretion

• All investigations, consultations, and follow-ups are subject to clinical evaluation and medical discretion.

• MD Health Center reserves the right to modify or limit services if deemed medically inappropriate.

6. Exclusions

• Emergency services, hospital admissions, surgical procedures, and medications are not included unless explicitly stated.

• Any additional tests or procedures outside the listed services will be charged separately.

7. Non-Refundable Policy

• The membership card fee is non-refundable and non-exchangeable, whether partially or fully utilized.

8. Right to Amend

• MD Health Center reserves the right to modify, update, or discontinue any service or term without prior notice.

• Updated Terms & Conditions will be available on the official website.
